چکیده

Results T wenty-six patients were prospectively evaluated. Two patients had normal TBV at admission. Twenty-four patients were hypervolemic with TBV (7.4 1.6 liters) increased by þ39 22% (range, þ9.5% to þ107%) above the expected normal volume. With diuresis, TBV decreased marginally (þ30 16%). Body weight declined by 6.9 5.2 kg, and fluid intake/fluid output was a net negative 8.4 5.2 liters. Interstitial compartment fluid loss was calculated at 6.2 4.0 liters, accounting for 85 15% of the total fluid reduction.

Management of Patients Admitted with Acute Decompensated Heart Failure.

BACKGROUND Hospital admission for the treatment of acute decompensated heart failure is an unfortunate certainty in the vast majority of patients with heart failure. Regardless of the etiology, inpatient treatment for acute decompensated heart failure portends a worsening prognosis. METHODS This review identifies patients with heart failure who need inpatient therapy and provides an overview ...

Fluid management strategies in heart failure.

In patients with chronic heart failure, fluid retention (or hypervolemia) is often the stimulus for acute decompensated heart failure that requires hospitalization. The pathophysiology of fluid retention is complex and involves both hemodynamic and clinical congestion. Signs and symptoms of both hemodynamic and clinical congestion should be assessed serially during hospitalization. Core heart f...

Biomarkers in acute heart failure.

The care of patients with acutely decompensated heart failure is being reshaped by the availability and understanding of several novel and emerging heart failure biomarkers. The gold standard biomarkers in heart failure are B-type natriuretic peptide and N-terminal pro-B-type natriuretic peptide, which play an important role in the diagnosis, prognosis, and management of acute decompensated hea...

Cardiorenal syndrome: new developments in the understanding and pharmacologic management.

Cardiorenal syndromes (CRSs) with bidirectional heart-kidney signaling are increasingly being recognized for their association with increased morbidity and mortality. In acute CRS, recognition of the importance of worsening kidney function complicating management of acute decompensated heart failure has led to the examination of this specific outcome in the context of acute heart failure clinic...
